Applicable to: Nonlicensed direct service personnel of the Department of Developmental Services or of its licensed facilities wishing to maintain certification
Description: Recertification program for unlicensed certified personnel to continue to administer medication to clients of the Department of Developmental Services or its licensed facilities under the supervision of a registered nurse.

Requirements
Prerequisites: Certification
Required Documentation: Initial certification in good standing, successful completion of Recertification process and exam. Recertification must be completed within the 90 day period prior to certification expiring.
Other Information: On-site practicum (Check list A) and On-site practicum form (Check list B) must be obtained from employer. Must obtainRecertification Registration Form from employer.
