*Call the Department of Public Health Legal Office at (860) 509-7600 to obtain background check/fingerprint cards. Fingerprints for all staff hired as of 9/1/2001 (if not already submitted) for FBI and State Police record review. A check or money order made payable to Commissioner of Public Safety for $24.00 for each set of fingerprints
