Must be signed by the organization ranking officer, and approved by the municipal Chief of Police or First Selectman

*  Bazaar Activity/Worker Sheet (CGR-2B)
*  Cow-Chip Raffle Activity Sheet (CGR-2C)
*  Duck-Race Raffle (CGR-2D)
*  Frog-Race Raffle Activity Sheet (CGR-2F).
